As the Clinical Care Coordinator you will work in conjunction with the treatment team in developing, coordinating, and disseminating continuing care plans with patients. Maintain up-to-date referral network.

• Duties
• Include: Meets with patients to research discharge planning needs as presented by the treatment team. Meets one-on-one with patients from admission until discharge at a minimum of one time per week to discuss case management. May be assigned to conduct educational lecture or group activity. Communicates with Business Development Department and referring provider to coordinate appropriate aftercare resources. Under the guidance of the Clinical Director, reviews and researches aftercare resources. Supports treatment team and patients in communication with referral and continuing care resources. Documents continuing care plans, communications and meetings. Prepares and distributes copies of patient schedules weekly as requested. Attends treatment team meetings as assigned. Ability to converse in English with others to give, take and process information; extensive use of telephone. Ability to maintain confidentiality of information, multi-task, coordinate projects and record/transcribe general and specific meeting minutes. Ability to work independently and collaboratively. Communicate with other staff to relay concerns and observations involving patient safety

• QualificationsEducation and
• Experience: Bachelors Degree in Behavioral Science or related field preferred. One years experience working in a behavioral health care facility preferred We are a Drug Free Company.
