The Clinical Care Coordinator is responsible for developing and providing high-quality therapeutic interventions and services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. Schedules may include after hours as needed by the program.
The THRIVE programs are new initiatives by Sacramento County Behavioral Health to provide services specifically for adults who are justice involved and meet criteria for specialty mental health services. Justice involved may include participating in mental health diversion, mental health court, parole/probation, etc. THRIVE Outpatient Program will serve the moderate level community members and be open a standard Monday-Friday. THRIVE FSP will serve the high intensity community members and operate seven days a week.
Schedule: Tuesday-Saturday (FSP), Sunday-Thursday (FSP)

Candidates must hold a BBS registration as an ASW, AMFT, APCC, LCSW, LMFT, or LPCC with at least one year of social service experience. A valid California driver's license, personal vehicle, and insurance are required for extensive local travel.
