Compensation: $40/hour or $125 per visit Affirmed Home Care is growing and seeking compassionate, skilled Licensed Practical Nurses (LPNs) to join our home care team. If you’re looking for flexibility, autonomy, and meaningful patient relationships, this is an opportunity to build a schedule that works for your life while delivering high-quality care in the community. What You’ll Do As a Per Diem LPN, you will provide skilled nursing care to clients in their homes throughout Westchester County. You’ll work independently while still having the full support of a responsive clinical team. Assignments include 12-hour day shifts, overnight shifts, and skilled nursing visits.

Active New York State LPN license and current BLS certification
Minimum 1 year of recent LPN experience in an acute care setting (home care experience preferred)
Eligibility to work in the United States
Recent physical exam (within 1 year)
Current PPD or QuantiFERON (within 1 year) or chest X-ray (within 5 years)
MMR immunizations within the last 10 years
