• Key
• Responsibilities: Assess, diagnose, plan, and intervene in actual or potential health problems for patients in the ambulatory care setting. Coordinate care for patients, families, and significant others, evaluating their response to treatments and interventions. Perform comprehensive patient assessments, including medical histories, vital signs, and physical examinations. Administer medications, treatments, and immunizations as prescribed. Order, interpret, and analyze diagnostic tests and laboratory results. Develop and implement individualized patient care plans in collaboration with physicians and other healthcare professionals. Educate patients and families on disease management, medication adherence, treatment options, and preventive care. Monitor patient progress and modify treatment plans as needed based on clinical findings. Maintain accurate, thorough, and timely documentation in electronic medical records. Participate in quality improvement initiatives and adhere to all regulatory and organizational standards. Provide telephone triage services, using clinical judgment to assess patient needs and direct them to appropriate levels of care.
