The Case Manager I is responsible for providing patient case management services, coordinating care plans, and ensuring smooth transitions for patients. They act as a communication link among various stakeholders to optimize patient outcomes and reduce care fragmentation.
A Master's Degree in Social Work is required for this position, but no prior experience is necessary. The role involves working collaboratively with a clinical team to identify patient needs and develop care plans.
