Service Area The RN will provide care in the following areas: Skokie Rogers Park Edgewater Lincoln Square Lake View Lincoln Park Downtown Chicago Surrounding areas
Completing the nursing process in the delivery of patient care, including assessment, planning, implementation, and evaluation of patient outcomes Total oversight of patients from admission to discharge Developing a plan of care based on findings Serving as the patient’s advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care Timely completion of all skilled nursing documentation Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs) Working within a multidisciplinary team approach, coordinating with PT, OT, ST, MSW, HHA’s, and office staff

Current Illinois State RN License CPR Certification Previous experience in nursing or other healthcare field experience Familiarity with medical software and equipment Ability to build rapport with patients Strong problem-solving and critical-thinking skills Ability to thrive in a fast-paced environment WellSky EMR knowledge is preferred but not required
