Provide detailed psychosocial assessments and counseling services to hospice patients and their families. Coordinate with an interdisciplinary team to develop care plans and manage bereavement programs.
Requires a Bachelor's degree in Social Work and a valid Texas state license to practice. A minimum of one year of healthcare experience is required, with a Master's degree and hospice experience preferred.
This position services the area South of I-10

Provides detailed psychosocial assessments and social work support services to patients and families. Assists with aspects of the hospice bereavement program as requested.
