Plan and coordinate individualized therapeutic support and services for patients with mental health and substance use issues. Conduct assessments, manage caseloads, and facilitate support groups while collaborating with a multidisciplinary treatment team.
Requires a Bachelor's degree in Social Work and eligibility for state licensure such as LSW, LAC, CAC, or RAC. CPR and de-escalation certifications are required, with one or more years of experience preferred.
