Pinnacle Hospice is seeking an After-Hours Hospice LVN to provide skilled, on-call nursing care outside of standard business hours, throughout the Sacramento Area (ROSEVILLE, ROCKLIN, AUBURN, LINCOLN, FOLSOM).
Current LVN license in good standing Strong clinical judgment and ability to work independently Excellent communication and problem-solving skills Compassionate presence in emotionally sensitive situations Valid driver’s license Reliable, insured vehicle in good working condition (per state and organizational requirements)

Hospice Afterhours RN/LVN (per diem/oncall) Be there when patients and families need you most Pinnacle Hospice is seeking an After-Hours Hospice LVN to provide skilled, on-call nursing care outside of standard business hours The After-Hours Hospice LVN provides professional nursing care to hospice patients during evenings, nights, and weekends This role ensures continuity of care, symptom management, and emotional support for patients and families from 5:00 PM - 8:30 AM The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care Respond to after-hours patient calls and urgent care needs Perform skilled nursing assessments and symptom management Provide emotional support and education to patients and families Implement and update patient plans of care as needed Communicate patient condition changes to the interdisciplinary team Collaborate with physicians and caregivers regarding patient needs Complete accurate and timely clinical documentation Uphold hospice philosophy, policies, and regulatory standards
Candidates must hold a current LVN license in good standing and possess a valid driver's license with a reliable, insured vehicle. Strong clinical judgment and the ability to work independently in emotionally sensitive situations are required.
