Requires an MD or DO degree, residency training, and board certification or eligibility in the designated discipline. Must possess a Lifestyle/Wellness Certification and a valid Utah medical license with full prescriptive authority.

In this role, you will provide clinical services to patients for preventing, treating, and managing chronic diseases and improving quality of life. This position includes a $15,000 sign-on bonus, and up to $20,000 in relocation assistance.
