How you’ll make an impact in this role Psychosocial Assessment
Evaluating complex emotional, psychological, and physical issues to determine the support needed for both patients and their families.
Interdisciplinary
Care
Planning: Collaborating with physicians, nurses, and therapists to develop and reevaluate individualized care plans from admission through discharge.
Patient
Advocacy: Ensuring the medical system respects patient rights and preferences, especially in difficult cases involving ethics, end-of-life decisions, or abuse/neglect.
Resource
Coordination: Connecting patients with community resources for financial assistance, housing, food, and long-term care placement (e.g., skilled nursing or rehab).
What minimum requirements you’ll need Licensure / Certification / Registration
Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Certified Social Worker preferred.
Education
Bachelor's degree in Social Work required.
Work
Experience: 3 years of experience preferred.
