Requires a Bachelor of Science in Nursing and a valid Registered Nurse license in the State of Michigan. Preference is given to candidates with six to twelve months of experience in community health or ambulatory care.

Under limited supervision and according to established policies and procedures, coordinates the implementation of the treatment plan identified by the health care provider in an ambulatory/outpatient setting. Performs assessment of clients to facilitate appropriate access and level of care required. Supports the designed process of care by triage, patient education, implementing consultations and referrals, participation in team conferences, discharge planning and telephone follow-up to clients.
1. Bachelor of Science degree in Nursing. 2. Six to twelve months of experience in ambulatory care or community health preferred. 3. License to practice as a Registered Nurse in the State of Michigan. Skills Required 1. Analytical skills necessary to evaluate patient status, health care procedures/techniques, and to monitor quality of patient care. 2. Interpersonal skills necessary to work productively with all levels of hospital personnel. 3. Verbal and written communication skills to communicate effectively with diverse populations including employees, patients and patient families.
