The Social Worker identifies social and emotional needs of participants and coordinates services between families, staff, and the community. They perform behavioral health assessments and collaborate with an interdisciplinary team to formulate treatment plans.
Requires a Master's Degree in Social Work and current LSW or LCSW licensure. Candidates must have at least one year of experience with frail or elderly populations and a current CPR certification.
The Social Worker shares the responsibility of identifying social and emotional needs of each participant. The position assists with arranging services to meet the needs of the participants, providing liaison between the participant, family, facility staff, and the community. Assess all participants behavioral health needs, gathers and analyzes information from available sources including family, agencies, friends and others. The Social Worker formulates and communicates treatment information to other members of the interdisciplinary team (IDT) and collaborates with the IDT on ongoing treatment and care decisions. The role also coordinates or provides services including individual, group, family, marital and/or group psychotherapy and serves as a resource to staff and community regarding psychosocial issues.
Experience: 2 years of hospital, counselling, community facing, or senior focused, social work or relevant fieldwork placement in mental health field or home care Experience working as a member of a an inter-disciplinary team in a hospital, nursing home, or community-based setting is preferable.
