Provide direct patient care and symptom management for hospice patients during evenings, nights, weekends, and holidays. Act as the primary liaison between patients, families, and physicians to ensure continuity of care.
Requires a current RN license, at least one year of nursing experience, and a valid driver's license with reliable transportation. CPR certification is mandatory, while experience in hospice or palliative care is preferred.
