Remote Case Manager RN (Costco Team) Location: Remote (Preference: Within 45 minutes of San Antonio, TX) Schedule: Monday–Friday, 8:00 AM–5:00 PM CST (occasional rotation to 10:00 AM–7:00 PM) No weekends, or holidays required
Join a dynamic team supporting Costco members as a Case Manager RN, where you’ll play a critical role in improving health outcomes through personalized, holistic care. This primarily remote role offers meaningful impact, work-life balance, and the opportunity to collaborate with a multidisciplinary care team. What You’ll Do Conduct telephonic (and occasional in-person) assessments of member health needs Develop and implement individualized care plans to improve outcomes Identify and address medical, behavioral, and social barriers to care Coordinate care across programs, providers, and resources Review claims/history to support effective case planning Engage members using strong communication and motivational interviewing skills Participate in case conferences and collaborate with clinical teams Required Qualifications Active, unrestricted multistate RN license (in state of residence) Willingness to obtain additional state licenses (company-paid) 3+ years of clinical RN experience 1+ year experience using Microsoft Office and web-based systems Ability to work scheduled hours with occasional evening rotation

The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
