RCT (RB-BHT) Care Coordinator

This position will serve as a member of the integrated care team and provide input and collaboration with other team members as needed. The role is essential in assisting members and/or legal guardians in obtaining the needed services and supports, identifying available housing, transportation, nutritional resources, and communicating with providers to promote coordinated and integrated whole person care.

On a typical day, you might

• Serve as a resource for accessing community-based resources and supports.
• Assist with coordinating services and support for members.
• Engage identified members into Care Coordination and utilize data and information from multiple sources to address members’ needs.
• Provide care coordination to assigned members with identified behavioral health, physical health, co-occurring, co-morbid or multi-morbid conditions.
• Provide education about all available services, natural and community support, treatment options, diagnosis, etc.
• Facilitate referrals for health-related services, community-based resources and social support services.

Qualifications

Required

• High School diploma/GED and six (6) years of experience working with BH/MH/SU/IDD population; OR
• Associate’s degree and four (4) years of experience working with BH/MH/SU/IDD population; OR
• Bachelor’s degree and two (2) years of experience working with BH/MH/SU/IDD population; OR
• Equivalent combination of education and experience.
• Must have a valid driver’s license and reliable transportation.
• Must reside within
• Trillium’s catchment area, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, or Wilson.
• Must be able to travel within catchment as required.
