The Hospice Aide will provide direct patient care under the direction of the Registered Nurse according to the established Plan of Care, assisting patients with safe transfers and ambulation. Responsibilities also include appropriately reporting changes, documenting care provided, practicing infection control, and ensuring compliance with all relevant laws and regulations.
Candidates must possess an unencumbered certification according to the applicable Nurse Aide Registry Licensure (Current CNA license) and have at least one year of experience as a Hospice Aide or Nursing Assistant in a relevant facility. Applicants also need a high school diploma or GED, a heart for compassionate care, high integrity, a valid driver's license, reliable transportation, and current auto liability insurance.

Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care. Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care. Practice accepted infection control principles.
