About the role

The RN Case Manager serves as the clinical anchor, coordinating personalized care plans and administering treatments for hospice patients. They act as the primary communication bridge between families, physicians, and community health partners.

Description Coverage Territory

  • Pontiac and surrounding areas
  • Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

What You'll Do As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most.

Requirements

What You Bring Active RN license in the state of employment Graduate of an accredited school of Nursing. One (1) + year Hospice, home health or related organization experience preferred. Compassion for patients and families navigating end-of-life care Strong clinical judgment and the ability to work independently in the field Excellent communication skills across care teams and families
