The Community Case Manager works directly with children, youth, and families to connect them with essential resources. They are responsible for documenting client progress and ensuring follow-through on treatment plans and referrals.
Candidates must have an Associate's degree with two years of experience or a Bachelor's degree. Experience in the mental health field and the ability to provide holistic therapeutic case management are required.
The pay for this position is $44,850-$58,908.9150 annually.
