The clinician will conduct community outreach to engage unhoused individuals and provide mental health assessments and crisis management. They are responsible for developing patient-centered treatment plans and coordinating referrals to ensure stability and independence for participants.
A licensed master's degree or higher in social work, counseling, nursing, or psychology is required, though those pursuing licensure or holding CASAC-Advanced are eligible. Experience with homeless populations and the ability to diagnose and treat behavioral health conditions are preferred.

Make referrals to necessary care and support services utilizing warm handoffs. Work closely with any existing care team(s) of individuals being served. Monitor engagement in services and status of permanent housing through continued connection to ensure stability, independence and wellbeing in their new community. Administer assessments related to mental health, addiction, and/or social determinants of health (training is provided). Crisis management and safety planning using the Columbia Protocol and other best practices. Completes all required documentation, in timelines in accordance with program standards. Clinician Qualifications: A licensed master’s degree or higher in social work, mental health counseling, nursing, or psychology is required (ex. LCSW, LMSW, LMHC, Licensed Psychologist, RN, etc.); individuals actively pursuing licensure or holding a CASAC‑Advanced certification are also eligible. Experience providing advocacy with homeless and/or precariously housed populations is preferred but not required. Experience as a provider and/or recipient of behavioral health, substance use and/or homeless services. Ability to diagnose, document and treat conditions through developing patient-centered treatment plans. Knowledge of: homeless resources within the community; treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff; community-based organizations and opportunities for continuing wellness development of participants; Intentional Peer Support models, best practices and methods of implementation; Broad-based health and related concerns that could be experienced by those who are unhoused or transitioning into housing. · Bi-lingual in Spanish and English preferred.
