Provide intensive, community-based case management to Veterans to help them transition from homelessness to permanent housing. Coordinate healthcare, benefits, and community resources while maintaining detailed documentation in HMIS.
Requires a Bachelor's degree in Social Work, Psychology, or a related field and at least two years of case management experience. Must possess a valid driver's license and reliable transportation for extensive community travel.
The typical working days for this position are Monday through Friday, mainly from 8:00 AM to 4:30 PM. Must maintain a high degree of flexibility as some days may require later hours.

The Veteran Case Manager provides intensive, community-based case management services to Veterans transitioning from homelessness into permanent housing through Department of Veterans Affairs (VA) Grant and Per Diem (GPD) and Healthcare for Homeless Veterans (HCHV) programs.
Using a Critical Time Intervention (CTI), Housing First, and trauma-informed approach, this position supports Veterans in achieving long-term housing stability, increasing self-sufficiency, and connecting to healthcare and community resources.
This position works extensively in the community and independently manages participant engagement, service coordination, documentation, and housing stabilization activities throughout Omaha and Lincoln.
This position is primarily community-based, with approximately 70% of work time spent in community settings including participant homes, apartments, partner agencies, public locations, and occasional field-based work from a vehicle.
The position also includes office-based responsibilities in a standard office environment with varying noise and temperature conditions. Employees may occasionally be exposed to outdoor weather conditions while conducting community visits.
Regular local travel throughout Omaha and Lincoln is required. Occasional evening or nontraditional hours may be necessary based on participant needs and program requirements.
This role requires flexibility, sound judgment, and the ability to effectively manage shifting priorities and high-stress situations.
