The Care Coordinator is responsible for providing cross-continuum care coordination, including psychosocial assessments and interventions. They collaborate with physicians and community colleagues to improve patient care and population health.
Requires a Master's Degree in Social Work from a CSWE accredited program and at least 5 years of recent clinical experience. A Licensed Social Worker certification is strongly preferred.

1. At least 5 years of recent clinical experience in a physician’s office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience. 2. Leadership experience a plus.
Master’s Degree in Social Work from CSWE (Council on Social Work Education) accredited program.
Licensed Social Worker strongly preferred.
