The Options Counselor performs health and functional assessments to develop community-based care plans for older adults and individuals with disabilities. They coordinate long-term care services and maintain detailed member records in compliance with regulations.
Candidates need at least two years of experience caring for persons 65 and older, including one year in a healthcare setting. A current Social Work license is preferred.
