The LPN provides comprehensive health oversight and hands-on care for program participants, including medication administration and wound care. They are responsible for maintaining accurate nursing documentation and collaborating with interdisciplinary teams to ensure resident well-being.
Candidates must hold a current DE RN/LPN license and CPR/First Aid certification. Additional requirements include a valid driver's license, FBI and Act 33/34 clearances, and completion of health screenings.

The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.
