The LPN provides comprehensive health oversight and hands-on care for program participants, including medication administration and wound care. They are responsible for maintaining accurate nursing documentation and collaborating with interdisciplinary teams to ensure optimal health outcomes.
Candidates must hold a current DE RN/LPN license and CPR/First Aid certification. Additional requirements include a valid driver's license, FBI and Act 33/34 clearances, and completion of physical and health screenings.

The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.
