Provide direct patient care, education, and support to patients and families facing life-limiting illnesses in their homes. Coordinate care with the Interdisciplinary Group to address physical, psychosocial, and spiritual needs to improve quality of life.
Requires a nursing diploma or ASN (BSN preferred) and a current Pennsylvania or compact nursing license. Candidates must have at least two years of recent med-surg experience and a valid driver's license with insurance.

1. Two year(s) of recent med-surg experience in an acute or long term care setting, rehabilitation, home care, or hospice setting. 2. Hospice Nurse experience preferred.
1. Graduate of an approved school of nursing. 2. RN Diploma, ASN required; BSN preferred.
1. Current license to practice nursing in Pennsylvania OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact. 2. Valid driver’s license and automobile insurance. 3. Provider BLS (American Heart Assoc/American Red Cross) preferred. 4. Certification in Hospice and Palliative Nursing preferred.
