Salary

Requires a High School Diploma or GED, with a bachelor's degree in a related field preferred. A minimum of two years of case management experience, ideally in behavioral health or substance use treatment, is preferred.
We are seeking a dedicated and organized Case Manager to support clients and families throughout the continuum of care. This role is essential to client success and includes coordinating services, communicating with referral sources and external providers, assisting with admissions processes, and supporting aftercare planning to ensure smooth transitions following treatment. Key Responsibilities Coordinate client care activities, including appointments, schedules, and treatment planning Communicate with referral sources and outside providers to ensure continuity of care Support the admissions process by verifying documentation, scanning records, and initiating communication with external providers as needed Conduct orientations with clients and families in collaboration with the primary therapist Provide case management services including transportation coordination, case management sessions, and support related to legal or employment needs Facilitate educational or skill-building groups to support clinical programming Develop and document comprehensive aftercare plans in collaboration with the treatment team and business development staff Provide aftercare documentation to appropriate external providers and ensure follow-up coordination Maintain accurate and timely documentation in the electronic clinical record system (CareLogic) Participate in treatment team meetings and maintain collaborative communication with counselors and staff Ensure compliance with confidentiality standards, licensing requirements, and program policies Complete required trainings and perform additional duties as assigned
Education High School Diploma or GED required; bachelor’s degree in a related field preferred Two years of previous case management experience preferred Experience Minimum of 2 years of case management experience preferred, ideally in behavioral health or substance use treatment Skills Strong organizational, communication, and interpersonal skills Ability to manage multiple tasks and meet deadlines Knowledge of community resources and referral processes
