  • Category: Nonprofit - Social ServicesJob Purpose The Case Manager plays a vital role in empowering Veterans and their families to achieve stability and independence. Through comprehensive case management, financial guidance, and hands-on support, the Case Manager helps ensure that Veterans experiencing housing crises and homelessness are quickly and successfully rehoused. This role is at the heart of creating hope, security, and lasting positive change in the lives of those who have served. Key Responsibilities Case Management & Client Support Deliver case management & supportive services to homeless Veterans; maintain accurate case files Determine participant service needs Develop Housing Identification and Stability Plans Develop and monitor Single Coordinated Care Plans Execute participation agreements with participants Create discharge plans with referral options Housing Stability & Counseling Provide housing stability counseling, including awareness and referral to rental, rent subsidy, and housing assistance programs Educate participants on supportive services and client rights, including fair housing, landlord-tenant laws, lease terms, rent delinquency, home maintenance, and financial management Referrals & Service Linkages Make program referrals and establish linkages with appropriate agencies, including: VA services and programs Provide linkages to Local shelters, GPD, and safe havens County Veteran Service Officers to inquire about VA benefits Local Veteran Agencies & non-profit organizations Vocational and rehabilitation counseling Employment, training, and educational assistance Healthcare and childcare services Protective payee and legal services Transportation assistance Daily living, emergency, and direct financial assistance Financial Counseling & Assistance Provide financial counseling and assist participants in developing personal budgets Request temporary financial assistance after Area Manager/Director approval ​​​​​​​Documentation & Compliance Maintain accurate, timely file records of client assessments, contacts, progress, and HMIS entries Outreach Attend local CoC meetings and applicable CoC sub-committee meetings Represent the organization’s SSVF Program at community events, networking opportunities, and public meetings Build partnerships with local organizations, both veteran and community-based Other duties as requested or assigned Key Performance Indicators 100% of participants have updated Housing and Coordinated Care Plans within 30 days of intake 100% of case files and participant documentation are complete and accurate in HMIS 80% of participants meet housing stability goals within the planned timeframe 90% of participants are connected to at least one resource within 30 days of intake 85% of participants develop and follow a personal budget plan 100% of client records updated within 48 hours of service delivery Zero compliance issues identified in internal or external audits QualificationsQualifications A bachelor’s degree in social services is required Minimum two years of experience in case management, life skills, as well as conducting assessments and counseling services An associate’s degree with 4-5 years of experience in case management, life skills, as well as conducting assessment and counseling services, is acceptable Experience providing services to veterans is strongly preferred Experience delivering services to SUD, COD, or homeless populations is also preferred Valid Wisconsin Driver’s License in good standing and current/valid auto insurance Other Skills & Abilities Ability to effectively communicate clearly and concisely in both oral and written formats High levels of professionalism with different stakeholders Skill in public relations and partnering practices Knowledge of the veteran recovery community and issues Must be flexible, maintain composure in difficult situations, and have a sense of humor Ability to relate to and interact with individuals from diverse cultural backgrounds Maintain confidentiality of written and verbal information Assist registered sex offenders and those with a history of violent crimes

Requirements

A bachelor's degree in social services and at least two years of case management experience are required, though an associate's degree with 4-5 years of experience is acceptable. A valid Wisconsin Driver's License and insurance are mandatory, with preference for experience serving veterans or populations with SUD/COD.
