The Cancer Care Liaison provides information, support, and assistance to patients and caregivers to overcome barriers to care. They collaborate with a multidisciplinary team to ensure high-quality, patient-centered care throughout the continuum.
A bachelor's degree and one year of related experience, or an equivalent combination of education and work, are required. Candidates must be able to read and interpret medical terminology.

The Cancer Care Liaison is responsible for providing information, support, encouragement, and assistance in overcoming barriers for patients and caregivers in collaboration with a multidisciplinary team to ensure access to high quality patient-centered care throughout the continuum. The Cancer Care Liaison works under the supervision of the assigned Coordinator.
1. Bachelor's degree and one year of related experience or an equivalent combination of education and work experience. 2. Ability to read and interpret medical terminology.
1. Experience in a patient care clinical or medical environment; oncology- related experience preferred. 2. Strong interpersonal and communication skills. 3. Computer experience with Microsoft Word, Microsoft Outlook, and Internet Explorer
8am-4: 30pm Weekend Requirements:
Registered Nurse, Licensed Practical Nurse, or Medical Assistant
Admissions Registered Nurse RN / Hospice
Registered Nurse RN / Hospice Visit PRN

Hospice Nurse (RN) – Day Shift / PRN
