Job Summary

Join our compassionate and collaborative team as a Registered Nurse (RN), where you will play a critical role in delivering high‑quality, person‑centered care across a variety of settings, including residential, outpatient, crisis, and community‑based environments. In this role, you will provide comprehensive nursing services including assessments, care coordination, medication management, and health education. You will collaborate closely with interdisciplinary teams to ensure clients receive holistic care that supports both their physical and behavioral health needs.

Key Responsibilities

· Perform nursing assessments and evaluate client physical health status using strong clinical judgment · Complete screenings, document medical histories, and record vital signs while coordinating follow‑up care · Monitor client health status and collaborate with treatment team members to ensure coordinated care · Develop action plans that address both physical and behavioral health needs · Provide disease prevention education, health promotion, and risk reduction strategies · Coordinate medical care including scheduling appointments and managing acute medical concerns · Administer and monitor medications in accordance with regulatory standards and physician orders · Educate clients and families on medication adherence and treatment plans · Monitor lab results and medication side effects and coordinate with providers as needed · Assist physicians or advanced practice providers with client exams and medical procedures · Coordinate medication access, including communication with pharmacies and indigent medication programs · Maintain accurate and timely documentation in accordance with organizational policies and regulatory requirements · Respond effectively to medical and behavioral health crisis situations · Support infection control, client hygiene, and overall health monitoring practices · Participate in interdisciplinary team meetings and care planning · Demonstrate commitment to organizational goals and perform other duties as assigned

Education, Experience, and/or Credential Qualifications

  • · Current Registered Nurse (RN) license in the state of practice · Willingness to become licensed in additional states as needed · Comfortable providing telehealth services ·

  • FQHC settings: BCLS certification required and ACLS certification preferred

Additional Qualifications

· Must have a valid driver’s license with an acceptable driving record and current auto insurance · Strong clinical judgment and critical thinking skills · Ability to work effectively with individuals experiencing behavioral health and substance use challenges · Strong communication and interpersonal skills · Ability to manage multiple priorities in a fast‑paced environment · Ability to maintain professionalism and composure in crisis situations · Proficiency with electronic health records and documentation systems

Physical Requirements · Ability to exert up to 10 pounds of force occasionally · Ability to perform repetitive hand, finger, and arm movements for typing and documentation · Ability to sit for extended periods with occasional walking or standing · Ability to meet the physical demands of a sedentary work environment
