Oversee medication delivery and administration for clients with severe mental illness while supporting the ACT fidelity model. Provide direct nursing care, assessments, and coordination with pharmacies and prescribers in a community setting.
Requires a BSN or Associate degree in Nursing and a current Colorado Registered Nurse license. Must possess a valid driver's license and have knowledge of psychiatric medications.
As a Registered Nurse Team under the supervision of the Director and Supervisor you will: Oversee the medication delivery process; including verifying medication, managing/auditing client MAR’s, completing/overseeing client pill planners when necessary and assist with medication delivery as needed; Provide nursing oversight to QMAP staff as needed to ensure safe medication administration practices; Periodically attend delivery route in order to provide services for clients in the community to include but not limited to: taking basic blood pressures, temperatures, weight monitoring, etc. Monitor medication changes for clients on the medication delivery route and collaborate with pharmacy and prescribers to reconcile client medication as needed; Provide basic nursing care for CARE Team clients in the community to include wound care, first aide, medication adjustment oversite, nursing assessments and other nursing duties as needed; Provide nursing assessments for CARE clients prior to prescriber appointments; receive and record prescriber orders; maintaining contact with pharmacies; obtain prior authorizations; facilitating lab work and reviewing results for the attention of the prescriber; Coordinate client care with Health Solutions Medical Center and outside providers when necessary; Participate/lead daily team meetings; Act as an active team member supporting the ACT fidelity model; Provide case management and medication education and training to a small caseload of clients diagnosed with severe and persistent mental illness; services could include transporting clients in company vehicle to attend medical appointments with client(s); Administering medication by injection as ordered by prescribers; Keeping accurate, complete and timely records, including electronic records; Participation in client staffing as needed; Willing and able to lead groups for CARE Team clients; Performing other job-related duties as assigned.
