Job summary

Buena Vista is seeking a Physical Therapist (PT) to join our home health team. The PT is responsible for providing physical therapy services to patients according to a written physician’s plan of care.

Duties & Responsibilities

  • Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care
  • Evaluate home environment
  • Assess for muscle strength, mobility, gait, ROM—potential for rehab
  • Direct physical therapy treatment
  • Instruct patients and families/caregivers in the use and care of therapeutic appliances
  • Determine priority needs for physical therapy
  • Report to physician patient’s reaction to treatment or changes in condition
  • Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient
  • Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences
  • Train patient in the use of prosthetic device
  • Identify patient and family/caregiver needs for other home health services
  • Prepare and submit clinical and progress summaries based on the attainment of goals
  • Participate in discharge planning for patient

  • Provide physical therapy consultation to home families/caregivers
  • Job requirements
  • Active CA PT license
  • Minimum of two (2) years of patient care experience; preferable in the home health setting
  • Possess and maintain current CPR certification.
  • Licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order

Pay range

$50.00 - $60.00 per hour

Requirements

Candidates must hold an active California PT license and a current CPR certification. A minimum of two years of patient care experience is required, preferably in a home health setting, along with a valid driver's license and insured vehicle.
