Psychiatric Nurse Practitioner (Hybrid- Hospital/Clinic) Westchester

This is a highly flexible, combined opportunity for a Psychiatric Mental Health Nurse Practitioner who is Medicaid credentialed to work across a dynamic blend of on-site clinical care and telehealth services. Requirements

• Conduct comprehensive psychiatric evaluations across outpatient clinic, hospital-affiliated, and telehealth settings
• Diagnose and manage a wide range of psychiatric conditions in adult and geriatric populations
• Provide medication management, including initiation, titration, and ongoing monitoring
• Deliver both in-person and virtual follow-up care, ensuring continuity and accessibility for patients
• Collaborate with interdisciplinary teams including physicians, therapists, and nursing staff
• Participate in care coordination for patients transitioning between hospital and outpatient settings
• Provide patient and family education regarding diagnoses, treatment plans, and medications
• Maintain accurate and timely documentation within EMR systems
