Job Summary Provides comprehensive social work services including discharge planning and coordination, patient education and counseling to assist in meeting the psycho social and discharge planning needs of patients and families. Enables patients to utilize medical care in order to achieve their optimal level of health.
Master's degree in Social Work required. NYS License in Social Work required. Minimum of one year of experience as a Social Worker required. Discharge planning experience Knowledge of Medicare Regulations Understanding of Managed Care Environment Excellent Interpersonal and Communication Skills Case Manager Certification a plus
