• Category: Nonprofit - Social ServicesThe Intake & Care Coordination Specialist is responsible for managing all referrals to the Permanent Supportive Housing Project and TAY Program including vacancies, waiting list, and interviewing applicants for determination of eligibility for the program. The Intake & Care Coordination Specialist is responsible for managing all aspects of the referral and housing databases, generating reports for funders, and gathering and disseminating housing resource information to stakeholders. The Intake & Care Coordination Specialist is responsible for coordinating care with other service providers and providing support to staff. In conjunction with other team members, the Intake & Care Coordination Specialist assists in ensuring that daily operations within the programs are consistent with TSI's mission statement and guiding principles as well as the agency’s Psychiatric Rehabilitation Approach Mission statement and values.
