The Discharge Planner manages the transition of mental health patients from the hospital to home or other care facilities. This includes coordinating with referral sources, conducting family therapy, and ensuring all medical records and discharge plans are completed.

A Bachelor's degree in Social Work or a related mental health field is preferred, along with at least one year of experience in a related discipline. Candidates must be able to pass a competency exam and obtain CPI training within 30 days of hire.
