Provides direct and indirect patient care through assessment, planning, intervention, and evaluation. Coordinates nursing care plans with patients, families, and interdisciplinary staff while communicating clinical changes to physicians.
Requires an Associate Degree in Nursing or Diploma, a current RN license, and BLS certification. Candidates must have at least two years of dialysis experience or a Certified Dialysis Nurse (CDN) certification.

Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develop nursing care plans in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharges planning process.
