Provide clinical medical social work services to pediatric patients and families undergoing weight management and bariatric treatment. Responsibilities include facilitating support groups, linking families to resources, and collaborating with an interdisciplinary team to improve patient health outcomes.
Requires a Master of Social Work (MSW) and a Licensed Social Worker (LSW) credential, with an LISW preferred. Two years of clinical social work experience is preferred.
The social worker in this role is a valued member of a passionate and highly skilled interdisciplinary team participating in the care of pediatric patients through 21 years of age receiving weight management treatment, which may include bariatric surgery. This involves provision of clinical medical social work services that includes supporting families with building and sustaining healthy lifestyle habits, coping and stress management, and overall goal setting. Other responsibilities include creating content for and facilitating monthly support groups and telehealth sessions, linking families to needed resources and support, and participating in monthly interdisciplinary team educational development sessions.
Summary: Provides developmentally appropriate, outcome-focused diagnostic and therapeutic service to patients and families in accordance with the scope of practice as defined in Chapter 4757 of the Ohio Revised Code, Section 4757.01 et seq. Practices professional social work and promotes advocacy in accordance with the NASW Code of Ethics for Social Work and maintains a current Ohio Social Work license.
Essential Functions: Functions as a member of the interdisciplinary team and provides Clinical Medical Social Work services to patients and their families to improve or maintain social, emotional, functional and physical health. Provides consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services. Participates in the development and delivery of education, research and continuous quality improvement. Education Requirement: MSW, required. Licensure Requirement: LSW, required. LISW, preferred.
