The Case Manager counsels and guides clients through individual case coordination and the utilization of community resources to promote health and wellbeing. Responsibilities include advocating for client needs, facilitating group sessions, and maintaining detailed clinical documentation.
Requires a Bachelor of Science degree in social services or a related field and a valid North Dakota Social Work license (LSW or LCSW). A valid driver's license is mandatory, and CPR/First-Aid certification is preferred.

JOB SUMMARY This position counsels and guides clients in the ShareHouse programs through individual case coordination by the location and utilization of resources. The role will assist clients with accessing programs, services and skills to promote health and wellbeing.

REPORTING RELATIONSHIPS The Case Manager does not supervise any positions.
& ESSENTIAL JOB FUNCTIONS Advocate for and address client concerns to promote successful outcomes for clients Address concerns related housing, finance, legal issues, insurance/treatment funding, and employment to provide resources and education with goal to empower and promote health/wellbeing Communication within agency departments as well as outside providers/referents for coordination during admission, providing care, and discharge planning. Areas include, but are not limited to, treatment/therapy, skills, training, education, physical/mental health, legal, etc. Assess clients for case management needs. Facilitate group sessions. Facilitate access to resources when required (e.g. Food Pantry, clothing vouchers, etc.) Complete required documentation; treatment plans, progress notes, group notes, discharge summaries, miscellaneous updates, etc. Provide and maintain reliable and prompt attendance, with the ability to work the schedule dictated by business necessity, to include evenings, on-call rotation and weekends as needed. Attend and participate in multidisciplinary staff meetings and consult with other professional staff to ensure appropriate treatment/programming for clients.
PERIODIC FUNCTIONS AND RESPONSIBILITIES Crisis management with in-house clients/families as needed Other duties as assigned.
Qualifications: Requires Bachelor of Science degree in social service, human service or related field of study
REQUIRED LICENSES/CERTIFICATES North Dakota Social Work license (LSW) or Licensed Certified Social Worker (LCSW) Must possess valid and current driver’s license and be able to be insured according to company insurance standards Prefer CPR and First-Aid certified
