Candidates must hold a current, unrestricted state RN license and have at least one year of clinical nursing experience. A valid driver's license, dependable vehicle, and the ability to travel and lift up to 50 pounds are required.

We are seeking a dedicated full-time Registered Nurse (RN) to join our Home Health team serving Arlington, Belmont, Watertown, Cambridge, Somerville, and surrounding communities. In this role, you will provide compassionate, one-on-one care in patients’ homes, supporting their independence and improving quality of life while building meaningful patient and family relationships. You will collaborate closely with physicians, caregivers, and interdisciplinary team members to develop and implement individualized plans of care that promote positive patient outcomes. Conduct comprehensive patient assessments and develop individualized plans of care. Perform initial evaluations and ongoing nursing visits based on patient needs and physician orders Monitor patient conditions and identify physical, psychosocial, environmental, and caregiver-related factors that may impact health outcomes. Coordinate care with physicians, therapists, social workers, and other members of the healthcare team. Communicate significant changes in patient condition and recommend appropriate interventions. Update and revise care plans as patient needs evolve. Provide education and support to patients, families, and caregivers. Maintain accurate, timely clinical documentation and ensure compliance with regulatory and company standards. Protect patient confidentiality and adhere to all HIPAA requirements.
Current, unrestricted state RN license. 1+ year experience in a clinical care setting as a nurse. Valid driver’s license and insurance and reliable transportation to perform job tasks Able and willing to travel within the branch/office coverage area. Must be able and willing to travel 50%. Ability to sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
