The Medical Case Manager provides patient-centered case management for unhoused and medically vulnerable individuals. Key duties include conducting needs assessments, developing care plans, and coordinating medical and behavioral health services.
A high school diploma is required, though a bachelor's degree in a healthcare field is preferred. Candidates should ideally have over 2 years of experience in ambulatory care or medical case management.
