In this role, you will play a key part in supporting patients as they transition from care by coordinating discharge plans, connecting them with aftercare resources, and collaborating with an interdisciplinary team. This position works closely with psychiatric inpatient and partial hospitalization (PHP) patients and their families to ensure a smooth and supportive discharge process. Windsor-Laurelwood Center for Behavioral Medicine, located in Northeast Ohio, specializes in psychiatric and chemical dependency treatment for children, adolescents, and adults. The facility provides a full continuum of inpatient and outpatient programs to meet patient’s individual needs. Assessment and referral service is available 24 hours a day, 7 days per week. Position Responsibilities Coordinate and lead discharge planning, including scheduling follow-up appointments and communicating with aftercare providers Collaborate with referral sources such as SASS, DCFS, and other agencies Build therapeutic relationships with patients and provide ongoing support during their stay Educate patients and families on mental health conditions and treatment options Participate in interdisciplinary team meetings and clinical staffings Advocate for patients and contribute to program development initiatives Perform other duties as assigned
