The Social Worker assesses the social, psychological, cultural, and financial situations of patients and their families. They act as advocates for patient rights and facilitate acute, rehabilitative, and long-term discharge processes.
A Bachelor's Degree in Social Work is required, with a BSW or LMSW license obtained within six months of hire. Preferred qualifications include 2+ years of social work experience, acute care case management experience, and ACM or CCM certification.
The Licensed Social Worker assesses the social, psychological, cultural, environmental, and financial situations concerning the patient and family. He/she is a patient/family advocate, promoting rights and dignity and striving to involve the patient/family in aspects of care as appropriate. The Licensed Social Worker also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care for the populations listed below.
