As a Foster Care Case Manager, you’ll play a vital role in supporting children, empowering foster families, and helping build stable, nurturing environments where youth can thrive.

This is a dynamic, relationship-driven role ideal for someone who is passionate about advocacy, collaboration, and making a meaningful impact every day. ________________________________________ What You’ll Do Champion Care for Youth & Families Serve as a trusted partner to children, foster families, and care teams—guiding them throughout the foster care journey Coordinate services, treatment planning, and resources to support each child’s unique needs Facilitate Child and Family Team Meetings and actively contribute to admission, treatment, and discharge planning Support & Empower Foster Families Provide ongoing coaching, training, and hands-on support to foster parents Conduct regular in-home visits to assess strengths, address needs, and ensure a safe, nurturing environment Help families navigate behavioral challenges using proven models like the Teaching Family Model Be a Consistent Advocate Maintain strong communication with biological families, social workers, and community partners Represent Children’s Hope Alliance in court hearings, treatment reviews, and external meetings Respond to urgent needs and provide crisis/on-call support when it matters most Ensure Quality & Compliance Keep accurate, timely documentation and case records using clinical management systems Monitor foster home compliance with licensing standards and conduct ongoing reviews Review medical administration records to ensure the safety and well-being of children ________________________________________ What Makes You a Great Fit You’re passionate about helping children and families succeed You build strong relationships and communicate with empathy and professionalism You thrive in a fast-paced environment where every day brings new challenges and rewards You’re organized and detail-oriented, with the ability to manage multiple priorities ________________________________________ What You’ll Need Bachelor’s degree in Psychology, Social Work, or a related field At least 2 years of experience working with children, families, or in a related human services role Qualified Professional (QP) designation (required) Ability to respond to urgent situations and participate in on-call rotation ________________________________________ Why Join Children’s Hope Alliance?
