This position carries a $1,000 dollar sign on bonus for qualified applicants with $500 paid at hire and $500 paid at the successful completion of a six-month introductory period*

The Care Coordinator plays a crucial role in ensuring that patients receive comprehensive and coordinated care throughout their healthcare journey.

This position involves collaborating with healthcare providers, patients, and their families to develop and implement individualized care plans that address medical, emotional, and social needs.

The Care Coordinator will monitor patient progress, facilitate communication among all parties, and advocate for patients to ensure they receive the necessary resources and support.

Minimum Qualifications

  • Bachelor's degree in human servies, social work, or a related field.
  • Experience in a healthcare setting, preferably in care coordination or case management.
  • Strong communication and interpersonal skills to effectively interact with patients and healthcare professionals.

Preferred Qualifications

  • Experience with electronic health record (EHR) systems.
  • Knowledge of community resources and support services available to patients.

Responsibilities
