Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways, which includes notifying physicians as appropriate.

Formulates a plan of care for individual needs, including patient population needs.

Participates in the planning of routine transitional health care needs (i.e. treatment options, patient placement options).

Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content.

Integrates education during the delivery of patient care.

Communicates patient's plan of care and significant changes to all members of the interdisciplinary team (using SBAR hands off reporting) and to the patient, family and significant others.

Makes independent clinical decisions based on knowledge drawn from education and experience.

Actively advocates for patient rights and identifies potential conflict.

Seeks counsel for resolution of conflict. #RN@DH Job Requirements Required: 1 year recent open heart OR critical care experience.

May consider training with one year of Telemetry or Step Down experience, if operational conditions exist.

Training positions require completion of basic critical care and dysrhythmia classes.

Associate degree in Nursing from an accredited school of nursing Verbal and written communication skills and computer skills Current California Registered Nurse license Current BLS, ACLS required (must be certified through the American Heart Association) PALS must be obtained within 12 months of hire into the position Preferred: 12-lead EKG and shock class completion preferred.
