Comprehensive primary care
Provide outpatient primary care services for patients across the lifespan (Family Medicine or Med-Peds scope).
Diagnose and treat acute and chronic medical conditions.
Conduct annual wellness visits, preventative screenings, and routine physicals.
Develop individualized treatment plans focused on long-term health outcomes.
Patient education & preventative care
Counsel patients on preventative health, lifestyle changes, and disease management.
Promote continuity of care through proactive follow-up and patient engagement.
Build strong relationships with patients and families within the local community.
Care coordination & collaboration
Collaborate with Advanced Practice Providers, Care Managers, Behavioral Health, and specialty teams.
Coordinate referrals, diagnostics, and follow-up care as needed.
Participate in a team-based, patient-centered care model.
Clinical documentation & compliance
Maintain accurate and timely documentation in the EHR system.
Ensure compliance with all clinical, regulatory, and quality standards.
Support quality initiatives and value-based care programs.

Required qualifications
MD or DO from an accredited medical school.
Board Certified or Board Eligible in:
Family Medicine or
Internal Medicine-Pediatrics (Med-Peds)
Active New Jersey medical license (or ability to obtain prior to start).
DEA license and CDS registration (or ability to obtain).
Strong commitment to patient-centered, culturally competent care.
