Provide coordinated and comprehensive nursing care to patients in their home settings, addressing physical, psychosocial, and spiritual needs. Utilize clinical judgment to manage acute, chronic, and life-limiting illnesses to improve patient independence and quality of life.
Requires an RN Diploma or ASN, a valid PA nursing license (or compact license), and a valid driver's license with insurance. Two years of recent med-surg experience in acute, long-term, or home care settings is preferred.

1. Two Years recent med-surg experience in an acute or long-term care setting, rehabilitation, home care, or hospice setting preferred.
RN Diploma, ASN required; BSN preferred.
1. Current license to practice nursing in PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact. 2. Valid Driver's license and automobile insurance. 3. Provider BLS (American Heart Assoc/American Red Cross) preferred. 4. Certification in specialty area of nursing preferred.
