Works collaboratively with physicians, staff and other health care professionals within his/her Conway Regional network, to provide care coordination across the health care continuum.
Is an integral member of the health care team who works to ensure Conway Regional’s patients are able to navigate through the health care continuum while improving the coordination of care and member/family experience.
Coordinates a wide range of community-based and healthcare support services for members.
Works with physicians, staff and other resources (including external to Conway Regional) in various settings to address specific quality improvement/performance improvement initiatives.
Collaborates with Conway Regional’s Population Health Care team on system-wide quality improvement/performance improvement initiatives.
Medical Assistant experience or Licensed Practical Nurse with a current, active license to practice in Arkansas required Proof of high school or equivalent or higher education Successful completion of Medication Administration Exam
Requires experience as a Medical Assistant or a current LPN license to practice in Arkansas. Must have a high school diploma or equivalent and successful completion of a Medication Administration Exam.
