• Schedule: Full Time We are seeking a compassionate and skilled Post-Acute Advanced Practice Provider (APP) to join our care team. In this role, you will support patients and families as they transition from hospital to post-acute settings, managing recovery and complex health needs with expertise and empathy.

Compensation

In compliance with New Jersey pay transparency laws, all clinician roles include a posted minimum base salary. This role offers a base starting salary at $125,000.00.
